Joint Affidavit of Two Disinterested Persons

– you’d need this if there’s a discrepancy among the names indicated in the documents.  i used to think that the person concerned shall be the one to issue the affidavit until i came across this one, as a requirement for filing of an insurance claim.

– i was also told that the two persons should not be related to the concerned person, should be older than him/ her, and should know the person for a very long time {since childhood, preferably}

– unlike other legal documents, i found it more difficult to get this one online in copy-paste.  i had to type everything  😛

JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

             WE, __, of legal age, Filipino, and with residence and postal address at ____ and ____ likewise of legal age, Filipino, and with residence address ____, after having been duly sworn to in accordance with the law, do hereby depose and state:

1. That we personally know ____ of legal age, Filipino and with residence and postal address at ____.  She is our ____ and have known her since childhood;

2. That unfortunately, when she enrolled for primary education, her name was erroneously entered as ____.  She has been using this name ever since and this appears in al of her education and employment records as well as in all her other documents;

3. That we hereby state under oath that the names ____, as appearing in her birth certificate and passport and ____, as appearing in  all her other records, refer to one and the same person;

4.  That we are executing this affidavit to support her request to have all her records amended to reflect the name ____, the name appearing in her birth certificate and passport.

5.  That I am executing this affidavit to attest to the truth of the foregoing facts and for whatever legal intents and purposes it may deem serve.
